DUTIES
- Provide skilled nursing care, under the direct supervision of the Registered Nurse
- Perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care, as necessary, to meet the client’s needs
- Perform direct and indirect care, then report care performed and client observations, through appropriate documentation
- Responsible, and accountable, for making decisions that are based on the individual's nursing experience and educational preparation
RESPONSIBILITIES
- Assist the Registered Nurse in carrying out the service plan
- Assist the Registered Nurse in performing specialized procedures, as directed by the Registered Nurse, under physician's orders
- Assist with the preparation, implementation, and continuing evaluation of the client service plan
- Recognize, and understand, the effects of social and economic problems upon clients and provide for the emotional and physical comfort and safety of clients
- Help teach the client the appropriate self-care techniques
- Foster cooperative effort among personnel, by understanding the functions of other persons involved in client services and by active participation in team and staff conferences
- Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning
- Prepare, and submit, clinical and progress notes, (not to include admission)
- Observe, record, and report, to the appropriate person, client symptoms, reactions, and changes including:* The general physical and mental condition of clients and signs and symptoms, which may be indicative of changes in condition
- Stresses in human relationships, between the clients and staff personnel, and between clients and their families and visitors
- Assist with the rehabilitation of clients, according to the medical service plan, by encouraging the interests and special aptitudes of the client and encouraging clients to help themselves using their own capabilities
- Further assist in rehabilitation by reporting any abnormality in the client’s range of motion, body mechanics, or body alignment.
- Actively participate in nursing organizations, promote, and participate in in-service programs
TREATMENTS AND PROCEDURES
- The Licensed Practical Nurse, with the education and proven clinical competence, may perform certain treatments/procedures, provided the Home Care Agency ’s policies and the State’s Regulatory Acts allow their performance.
- Only advance LPNs, with documented skill in performing these functions, may:* Administer medications
- Hang I.V.s premixed with medications
- Hang hydrating fluid without medications
- Start peripheral I.V.s
- Flush peripheral lines with saline or heparin solutions
- Inspect insertion sites, change dressings, and remove peripheral I.V. needles/catheters
- Calculate and adjust flow rates
- Check blood with a Registered Nurse
JOB CONDITIONS
- The ability to communicate well, both verbally and in writing is required.
- The ability to access clients’ homes which may not be routinely wheelchair accessible is required.
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
- Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of clients and the turning of clients.
EQUIPMENT OPERATION
* Utilization of calculator, multi-line telephone, copy machine, basic medical equipment
COMPANY INFORMATION
* Has access to all client medical records, personnel records and client financial accounts which may be discussed with the Executive Director, Director of Reimbursement, Employee Relations, and Quality Assurance department staff.
ENVIRONMENT AND PHYSICAL REQUIREMENTS
- Contact with clients under a wide variety of circumstances.
- Care is provided in client’s living environment with varying situations, including private homes, independent or assisted living or in an institution such as a hospital or long-term care facility.
- May be required to respond to emergency situations.
- Travel required.
- Position includes tasks that have the potential for exposure to blood/body fluids/tissues and other potentially infectious material such as body fluids/tissues.
PHYSICAL REQUIREMENTS
- Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
- Must be able to lift up to 50 lbs.
- Corrected visual and hearing acuity required.
QUALIFICATIONS
- Graduation from an accredited School of Practical Nursing
- Must be licensed, in Hawaii, as a Licensed Practical Nurse
- One, or more years, of experience, in community/Home Care Agency or in a hospital setting. Home care experience preferred
- Must have a criminal background check
- Must have current CPR certification. Online certification is not accepted.
- The ability to communicate well, both, verbally, and in writing, in English, is required.
