Job Summary: The Chaplain is responsible for providing spiritual support and counseling for HCP patients and their families in regard to end-of-life issues. The Chaplain is a member of the HCP Interdisciplinary Team. The Chaplain is responsible for coordinating spiritual care with community clergy, as appropriate, for HCP patients and families. This position is considered Full Time but will work 32 hours per week. Qualifications:

• A Bachelor’s Degree (Master's Degree preferred) in Divinity and/or all required credentials and ordination for a particular religious denomination. Clinical Pastoral Education (2 units) is required.
• Board Certification preferred but must be completed within 5 years of hire date.
• Possesses an understanding of various religious faiths and their beliefs regarding death and dying.
• Experience providing spiritual support to individuals in a health-care setting.
• Good communication skills and ability to functions as part of a team.
• Support of the hospice philosophy and mode of care.
• Access to own transportation with current driver’s license.
• Ability to sit and stand for prolonged periods of time.
• Ability to bend, lift, push and carry boxes weighing 20 pounds.
• Eyesight corrected to 20/20 or ability to accurately visualize numbers.
• Ability to negotiate steps.

• Will participate as member of the Interdisciplinary Team to develop a Spiritual Plan of Care for each HCP patient and their family.
• Will make home visits or telephone contacts, as requested by patient/family or recommended by the Team, to address spiritual issues with a HCP patient and/or family member.
• Will coordinate spiritual care with patient’s community clergy; i.e. priest, minister, rabbi, when requested and appropriate.
• Will make other community referrals, as appropriate, in order to best meet the patient and/or family’s spiritual needs.
• Will complete all required documentation for patient visits, telephone contacts and referrals. All documentation will become part of the patient’s medical record.
• Will educate the team regarding spiritual issues and their impact on the patient and family.
• Will provide in-service education and/or other types of spiritual programs, as appropriate, for HCP Team or community groups.
• May preside over funerals and/or Memorial services for HCP patients where appropriate and requested by the patient or family.
• Will participate in HCP’s annual Memorial Service and other community programs as assigned.
• May be requested to contact or visit a bereaved family member when spiritual concerns are an issue in their bereavement.
• May be requested to participate in a bereavement support group or education programs to address specific spiritual topics in bereavement.
• Perform other duties as assigned.
